• We require notification prior to shipment of all live specimens to ensure appropriate personnel and suppies are available.

• Complete contact and billing information is required.
• Please fill out the facility and fish information sections of the Pathology Submission Form as completely as possible. The historical and clinical information provided will be used along with the test results to make specific recommendations for control or treatment of the problem. Complete responses will help the laboratory provide you with optimal test results and recommendations.
• The second page of the Submission Form is designed to be duplicated as needed, to provide information on each individual fish or group of fish submitted.

• Lab Test Results
• • Histopathology cases typically take 2-3 weeks but may vary depending upon sectioning requirements or special stains.
  • An electronic and written (mailed) report will be provided

• Billing Information
• • An invoice will be sent to the billing address when the case is completed. Payment can be made by purchase order, check or credit card (Visa, Master Card or Discover Card). For security purposes, please do not send payment or credit card information with specimens. If credit card payment is requested, we will provide a separate form for credit card information to be faxed or mailed directly to our accounting office.
